How Hyperaldosteronism Medications Work

Hyperaldosteronism is a condition characterized by the excessive production of aldosterone, a hormone that regulates electrolyte balance and blood pressure. The primary goal of medication treatment in hyperaldosteronism is to reduce aldosterone levels and alleviate symptoms.

Mechanism of Action

  • Spironolactone (Potassium-sparing diuretic): Blocks the action of aldosterone at the mineralocorticoid receptor, reducing sodium reabsorption in the kidneys and potassium excretion. This leads to increased potassium levels and decreased blood pressure.
  • Eplerenone (Mineralocorticoid receptor antagonist): Similar mechanism of action as spironolactone, but with a more selective binding affinity for aldosterone receptors.
  • Aldosterone antagonists (e.g., Amiloride): Inhibit the sodium-potassium pump in the kidneys, reducing sodium reabsorption and potassium excretion.

Effectiveness and Alternatives

The gold standard treatment for hyperaldosteronism is surgery (adrenalectomy), but medications are often used as an alternative or adjunctive therapy. Medications can provide symptom relief and reduce the need for surgical intervention in some cases.
